Description

Ejaculate karyotyping (FISH) and DNA fragmentation (TUNEL) are genetic tests of sperm used to identify the causes of male infertility, miscarriage, and IVF failure. They allow for the assessment of chromosomal abnormalities and DNA damage in sperm.

FISH (Fluorescence in situ hybridization) is a highly specific cytogenetic test that visualizes individual chromosomes in sperm using fluorescent probes. It is aimed at determining whether the sperm has the correct chromosome complement, which is especially important in cases of unexplained infertility, recurrent miscarriages, or failed IVF attempts.

The test covers a panel of five chromosomes:

  • 13, 18, and 21 are the most common autosomal trisomies (associated with Patau, Edwards, and Down syndromes);
  • X and Y are the sex chromosomes, abnormalities of which can be associated with infertility or hereditary syndromes.

Normally, sperm should contain:

  • one signal from each of the analyzed autosomes (13, 18, and 21);
  • one signal from one of the sex chromosomes (X or Y).

An increased percentage of sperm with aneuploidies (abnormal chromosome number) may indicate:

  • a genetic cause of infertility;
  • risk of abnormal fetal development;
  • likelihood of miscarriage or low IVF success.

The TUNEL (Terminal deoxynucleotidyl transferase dUTP Nick End Labeling) method determines the level of sperm DNA fragmentation, i.e., the presence of breaks in the DNA strands. This test assesses the quality of the genetic material, regardless of the number of sperm in the ejaculate. Increased fragmentation is associated with decreased fertility, delayed embryo development, miscarriages, and reduced success rates of assisted reproductive technologies (ART).

The combination of FISH and TUNEL allows for a comprehensive assessment of the genetic status of sperm and the identification of causes of male infertility, even with normal semen test results.

When and who needs the test?

  • If male factor infertility is suspected.
  • In case of abnormalities in the spermogram.
  • In case of habitual miscarriage in a couple.
  • In case of a chromosomal abnormality in the fetus during previous pregnancies.
  • After repeated unsuccessful IVF/ICSI attempts.
  • If a balanced chromosomal rearrangement is detected in the man.
  • To assess the risk of transmitting unbalanced translocations to offspring.
