What is Health Screening 40+?
The National Health Screening 40+ Program is available to every Ukrainian aged 40 and over and includes basic examinations for the early detection of diseases. You can undergo screening at Synevo free of charge in one visit. To do this, you need to receive a payment from the state to your Diia.Card.
- Lipid profile (total-C, HDL-C, LDL-C, TG, non-HDL-C)
- Glycated hemoglobin (HbA1c)
- As clinically indicated:
- Serum creatinine + eGFR and urine ACR
- Blood electrolytes
- Electrocardiography (ECG)
- Measurement of blood pressure, pulse, weight, height, waist circumference, and body mass index (BMI)
- Collection of data on risk factors and symptom screening
- Preventive counseling (lifestyle, nutrition, physical activity, harmful habits)
How to sign up for Health Screening 40+?

Receive an invitation in Diia
If you are over 40 years old, 30 days after your birthday you will receive an invitation in the Diia app to undergo the annual Health Screening 40+. If you do not use the Diia app, you can still join the program via the Administrative Service Center (CNAP).

Confirm participation and receive funds
Accept the invitation in the Diia app or contact a CNAP. You will receive a Diia.Card or use an existing one. Within 7 days, funds will be credited to the Diia.Card to pay for the screening. You will be able to pay with the card during your visit.

Choose Synevo for screening
Regardless of your place of residence or registration, you can undergo screening at Synevo. You can book an examination via the website, on a walk-in basis at the branch, or via the Synevo information service: 0 800 60 55 00. Payment is made with a Diia.Card.

Complete the examination in one visit
You can complete the basic health screening in one visit. During the visit, the doctor will conduct an examination and consultation. This takes approximately 60–90 minutes. Test results will be sent to your email address and will be available in your personal account on the Synevo website.
Important information
- Funds on the Diia.Card (UAH 2000) can be used exclusively for the “Screening 40+” program*
- Funds are credited within 7 days
- The program is available only if you have an invitation in Diia
- If you do not have the Diia app — contact your nearest Administrative Service Centre. *Payment for services is made using the Diia Card with budget funds. The services are provided by Synevo Ukraine LLC within the framework of the project for conducting health screening for persons aged 40 and older in accordance with the Resolution of the Cabinet of Ministers of Ukraine dated December 10, 2025 No. 1652 “Some issues of conducting health screening for persons aged 40 and older”.
