Description

PCR testing of semen for Chlamydia trachomatis allows detection of the pathogen's DNA in cases of urethritis, epididymitis, or male infertility. This test is essential for a comprehensive assessment of infectious factors affecting fertility.

Chlamydia trachomatis is one of the main causes of non-gonococcal urethritis, chronic inflammation of the scrotum, and male infectious infertility. In many cases, the infection is asymptomatic or has minimal symptoms (e.g., short-term burning, slight discharge), but it triggers chronic inflammatory changes in the epithelium of the urethra, epididymis (epididymitis), prostate (prostatitis), and even the vas deferens.

The bacterium can penetrate the spermatogenic epithelium, disrupt the microenvironment in which sperm mature, and trigger an immune response with the formation of antisperm antibodies. As a result, this can reduce:

  • ejaculate volume (due to concomitant prostatitis or vesiculitis);
  • sperm concentration (oligospermia);
  • sperm motility (asthenospermia);
  • morphological quality (teratospermia);
  • DNA integrity (increased fragmentation, which reduces the chances of fertilization).

Sperm PCR testing is extremely valuable in such situations, as it allows for the detection of Chlamydia trachomatis DNA directly in the ejaculate, even if other biomaterials (urine, pelvic swab) are negative. This is especially relevant for men with unexplained infertility, chronic pelvic pain, or when planning to participate in assisted reproductive technology programs (IVF, ICSI), where the presence of the pathogen significantly reduces the effectiveness of the procedures and increases the risk of infection of the partner or embryo.

Considering the infectious factor in sperm evaluation is an essential component of a comprehensive approach to men's health.

When and who needs the test?

  • Men with suspected infectious infertility.
  • In case of sperm abnormalities (oligospermia, asthenospermia, teratospermia).
  • Before IVF/ICSI or sperm donation.
  • For chronic pain in the scrotum, pelvis, or prostate.
  • As an additional test after a negative urine or urogenital swab PCR result.
