Description

PCR testing of cerebrospinal fluid for Toxoplasma gondii is a method for detecting parasite DNA when CNS lesions are suspected. It provides a qualitative result (detected/not detected). It is used in immunosuppressed patients, HIV/AIDS patients, and when congenital toxoplasmosis is suspected.

Toxoplasma gondii is an intracellular parasite with high neurotropic activity, capable of penetrating the blood-brain barrier and infecting brain structures. In immunocompetent patients, infection is usually asymptomatic or mild, but in immunosuppressed conditions (HIV/AIDS, cancer, post-transplant), T. gondii can cause severe damage to the central nervous system.

Neurotoxoplasmosis is characterized by:

  • Encephalitis, meningitis, meningeal lesions.
  • Seizures, impaired consciousness, cognitive deficits.
  • Focal neurological symptoms (paresis, visual and speech impairment).
  • Chorioretinitis and other forms of ophthalmotoxoplasmosis.

Toxoplasmosis is most dangerous in newborns infected in utero, causing CNS damage, hydrocephalus, and epileptic seizures. Polymerase chain reaction (PCR) for Toxoplasma gondii in cerebrospinal fluid is a direct method for detecting pathogen DNA in cerebrospinal fluid, allowing confirmation of active parasitic infection of the central nervous system.

This method is extremely informative in clinical situations where:

  • Serological tests yield unclear results (IgG+/IgM− or no antibodies at all in severe immunodeficiency).
  • There are clinical signs of neuroinfection, but no confirmed etiology.
  • It is necessary to differentiate toxoplasmosis from other central nervous system infections (CMV, VHF, cryptococcosis, etc.).

When and who needs the test?

  • For patients with HIV/AIDS with suspected encephalitis.
  • For focal neurological symptoms, fever, and altered consciousness associated with immunosuppression.
  • For newborns with clinical signs of CNS damage with suspected congenital toxoplasmosis.
  • For a comprehensive assessment of neuroinfections of unknown etiology, in conjunction with MRI, serology, and clinical data.
