Description

Ovarian tumor marker (CA 125) is a tumor marker whose levels may increase in ovarian cancer, as well as in gastrointestinal, renal, and lung pathologies. It is used for diagnosis, treatment monitoring, and recurrence detection. It is recommended to be tested in combination with HE4, CA 19-9, CA 50, and other markers.

CA 125 is a high-molecular-weight glycoprotein (mucin-16) synthesized by cells of mesothelial origin, particularly the epithelium of the ovaries, fallopian tubes, endometrium, pleura, peritoneum, and pericardium. Its physiological role is to moisturize and protect mucosal surfaces.

However, in pathological processes, especially malignant ones, CA 125 blood levels can increase significantly. The highest diagnostic value of CA 125 is observed in patients with epithelial ovarian cancer – this marker is considered the gold standard for monitoring the progression of the disease, monitoring the effectiveness of treatment, and detecting relapses. At the same time, CA 125 levels can also be elevated in other oncological processes (pancreatic, breast, lung, and colon cancer), as well as in non-oncological conditions: endometriosis, uterine fibroids, pelvic inflammatory disease, pregnancy (especially in the first trimester), menstruation, and liver cirrhosis. Due to the low specificity of this indicator, the results should be interpreted in the context of the clinical picture and in combination with other tumor markers (HE4, CA 19-9, CA 50, CA 72-4, PEA, and BRCA mutations). For example, the combined determination of CA 125 and HE4 allows for the use of the ROMA index (Risk of Ovarian Malignancy Algorithm), a tool for assessing the risk of epithelial ovarian cancer in perimenopause and postmenopause.

This marker can also be used in dynamic monitoring of patients with a familial or hereditary risk of developing ovarian cancer.

When and who needs the test?

  • women with suspected ovarian cancer;
  • to evaluate the effectiveness of therapy and chemotherapy;
  • for the early detection of recurrences or metastases;
  • in the presence of nonspecific pelvic symptoms;
  • in patients with a high familial or hereditary risk of developing ovarian cancer.
