Description

Triiodothyronine is one of the thyroid hormones that plays a key role in the body's metabolism. The total level reflects the combined amount of protein-bound and free triiodothyronine.

Testing is necessary if you have a low TSH level, especially when free thyroxine levels are normal, and/or if there are symptoms of an overactive thyroid. For an accurate assessment of your condition, it is important to evaluate total T3 together with free T3, T4, and TSH.

Total T3 reflects the combined concentration of both free (unbound) and protein-bound triiodothyronine in the blood. T3 is a thyroid hormone and an iodinated derivative of the amino acid tyrosine.

About 20% of circulating T3 is secreted by the thyroid gland under the control of TSH, while the remaining 80% is formed by peripheral conversion of T4 — primarily in the liver and kidneys. Approximately 30% of T4 is metabolized into T3 and reverse T3 (rT3), the latter being an inactive form that does not exert hormonal effects or suppress TSH secretion.

T3 is around three times more potent than T4 in terms of hormonal activity. More than 80% of circulating T3 is bound to thyroxine-binding globulin (TBG). Increases in TBG levels lead to higher total T3 concentrations.

When and who needs the test?

  • Diagnosis and monitoring of T3 thyrotoxicosis.
  • Assessment of thyroid function (manifest or subclinical thyrotoxicosis).
  • Differential diagnosis of thyroid disorders.
  • Monitoring the effectiveness of thyrotoxicosis treatment, as well as high-dose replacement or suppressive therapy with T4-based medications.
  • Detection of pseudo-thyroid dysfunction syndrome in somatic illness (low T3 levels due to impaired conversion of T4 to T3).
