Attention! At the request of the NHSU, to take the test according to the package of medical guarantees at the laboratory center, you should present an original identification document (passport of a citizen of Ukraine, international passport, application 'Дія' (Diia), permanent/temporary residence permit, pension certificate of a new type (a photo, full name and full date of birth should be contained in the document), for children under 14 years of age – birth certificate and person identifying document of one of the parents or guardian) and have an electronic referral from a doctor.
