Description
Copeptin (C-terminal pro arginine-vasopressin, CT-proAVP) is a marker for assessing the activity of vasopressin (antidiuretic hormone, ADH) synthesis in response to changes in water balance and acute hemodynamic stress. The prohormone vasopressin is cleaved into ADH, neurophysin II and copeptin.
Copeptin is secreted in equimolar concentrations along with ADH, is more stable in plasma, therefore, is considered as a surrogate marker of ADH when assessing water balance disorders. Both copeptin and ADH respond to osmotic stimuli and their synthesis is increased in response to water deprivation.
When and who needs the test?
- differential diagnosis of patients with water balance disorders, including diabetes insipidus in combination with osmolality and hydration status
- prognosis and risk assessment of cardiovascular diseases (acute coronary syndrome, congestive heart failure, acute ischemic stroke) in combination with other cardiac markers
