Description

Hazelnut allergy is a type of food allergy to nuts, in which the allergen is hazelnuts. However, a reaction to hazelnuts (domesticated hazelnuts or filberts) is considered the most common food allergy. Hazelnut allergy is most often seen in patients with tree pollen allergies. Symptoms of hazelnut allergy can generally be similar to oral allergy syndrome: itching and burning in the mouth, tingling of the tongue, swelling of the tongue, a rash around the mouth, and peeling of the vermilion border of the lips. This test is used to diagnose hazelnut allergy and, in conjunction with sensitivity testing to other pollen antigens, to determine a person's allergy status.

Hazelnut allergy is one of the most common forms of tree nut food allergy, arising from an IgE-mediated immune response. It develops in both children and adults, frequently in the context of atopy or pollen sensitization.

Hazelnut contains several clinically significant allergens that differ in stability and the severity of reactions they provoke. Thermolabile proteins (Cor a 1, Cor a 2) typically cause mild local symptoms – oral allergy syndrome: itching, burning sensation in the mouth, and swelling of the lips and tongue. Such reactions are characteristic of patients sensitized to birch pollen due to cross-reactivity.

Thermostable proteins (Cor a 9, Cor a 8) are resistant to heat and digestion and may therefore trigger systemic reactions, including generalized urticaria, angioedema, bronchospasm, gastrointestinal symptoms, and anaphylaxis. These components are the primary determinants of severe allergic reactions.

Symptoms may appear immediately or within several hours of exposure. Reactions can occur not only to whole hazelnuts but also to products containing hazelnut traces – including chocolate, confectionery, ice cream, and yoghurt. Sensitization to hazelnut allergens may be associated with cross-reactivity to other tree nuts, peanuts, soy, almonds, peach, and cherry.

When and who needs the test?

  • Testing is indicated for patients with suspected hazelnut allergy, particularly when symptoms arise following its consumption — including itching and burning in the oral cavity, swelling of the lips and tongue, urticaria, gastrointestinal manifestations, or respiratory symptoms.
  • Recommended for individuals with pollinosis (especially those sensitized to birch pollen), in whom cross-reactive responses to hazelnut and the development of oral allergy syndrome are possible.
  • Indicated for patients with a history of systemic allergic reactions to nuts, to assess the risk of severe reactions including anaphylaxis.
  • Appropriate for children and adults with atopic conditions (atopic dermatitis, bronchial asthma, allergic rhinitis), as well as in cases where differentiation between primary food allergy and cross-reactive pollen-food reactivity is required.
